Please be aware of the following Policies:

Policy 1

Appointments may be scheduled, rescheduled, cancelled by phone or by e-mail Monday through Friday. Failure to give notice for any cancelled appointment at least 24 hrs in advance will result in a full charge of the session that was reserved for you.

Policy 3

Self-pay rates are $150.00 for the initial session and $120 for each session after. Contact me for more information.

Policy 4

Methods of accepted forms of payment include bank debit and/or credit cards, such as Visa, MasterCard, Discover, and American Express, as well as the following secure, online payment services: PayPal, Cash App, or Venmo.

Policy 5

As of right now, I am not a preferred provider for any insurance companies, so in most cases the benefit will be paid at the out-of-network rate and the reimbursement will be sent directly to you by your insurance provider. Therefore, even when your appointments are covered by insurance, payment for sessions is required in full at the time of service. Some insurance companies do not pay for out of network therapy services so please be sure to check with your insurance claims department.

Policy 6

Payment is due at time of services are rendered. Client is responsible for the amount of the session and any remaining balances unpaid by your insurance carrier if you choose to utilize the out-of-network rate.
